Important!

REMEMBER THAT YOU WILL ONLY FILE YOUR CLAIM.

IT IS ESSENTIAL FOR YOUR REGISTRATION THAT IT BE PROPERLY COMPLETED.

HOW TO FILE YOUR CLAIM

1. Fill in the form (CLAIM FORM) in clear print, completing all sections including your mobile phone number, which is essential.

2. In the THIRD/THIRD point in the form, check the box for CONSENT: YES

3. In order to register your claim, you can do the following:

a. In Mallorca:

·  Book an appointment HERE to register your claim.
You need to bring the original document and a photocopy

·  Remotely:

–  You can send your claim by post (by ordinary or registered post or registered fax…)
–  By fax to 971763546.

b.    In Ibiza:

·  Book an appointment HERE to register your claim.
You need to bring the original document and a photocopy

·  Remotely:

– You can send your claim by post (by ordinary or registered post or registered fax…)
– By fax to 971398200.

c.     In Menorca

· Book an appointment HERE to register your claim.
You need to bring the original document and a photocopy

·  Remotely:

– You can send your claim by post (by ordinary or registered post or registered fax…)
– By fax to 971356525

4.    Please be on time. In order to avoid overcrowding in the TAMIB offices, only one person per registration will be allowed in.

ONCE YOUR CLAIM HAS BEEN SUBMITEED

1. Within 48 hours you will receive a text message (SMS) to your mobile phone with the date and time of the mediation appointment.

2. If you need to give power of attorney to a third party (acknowledgement or representation) you must book an appointment by phone to 971763545 (Mallorca), 971398271 (Ibiza) or 971356554 (Menorca).

3. The day of the mediation appointment:

a. You must abide by all the required protective measures. The use of mask and gloves is recommended.

b. Punctuality is required. In order to avoid overcrowding and possible delays in the mediation, no grace period will be given.

c. In order to avoid overcrowding in the Tamib offices, only those that have been summoned will be allowed in the premises, together with their legal representatives if so required.

d. When summoned, you must produce your original ID card or passport (no photocopy or photo on your mobile phone will be accepted)

1. To file your claim, you can do the following:

a. In Mallorca:
·  By fax to 971763546.
·  By post, sending an ordinary, registered post or registered fax.
·  By email to: registromallorca@tamib.es

Please make sure you give your mobile phone number, as the summons will be sent by text message (SMS). The express consent clause, in which YES has been ticked, must also be included.

b. In Ibiza:
·  By fax to 971398200.
·  By post, sending an ordinary, registered post or registered fax.
·  By email to: registroibiza@tamib.es

Express consent clause:
In accordance with the provisions set up in the current legislation on Personal Data Protection and the EU Regulation (2016/679 of27th April 2016 (GDPR), your personal data will be processed under the responsibility of FUNDACIÓ TRIBUNAL D’ARBITRATGE I MEDIACIÓ DE LES ILLES BALEARS (TAMIB).

Purpose: the provision of our services and handling of your request for arbitration, conciliation or mediation. Sending text messages to your mobile phone informing you of the date and time of the appointment for the mediation. If you have not received a text within 48 hours you must go to TAMIB to withdraw your summons CONSENT YES □ NO □.
Retention: as long as there is mutual interest to do so, in accordance with the applicable legal deadlines. Legitimation: consent by the interested party o implementation of a contract. Recipients: data will not be transmitted to third parties unless legally required to do so, and to collaborators needed to ensure the proper provision of services. You can the right to access, rectify or delete your data, as well as those of limitation, portability and opposition to its processing. You can do so by means of written notification sent to  Avenida Conde de Sallent 11, 2ª Planta, 07003  Palma de Mallorca, as mentioned above, or by sending an email to info@tamib.es. More information at www.tamib.es. If you believe that  processing does not abide by  current regulations, you may lodge a complaint with the Supervisory Authority at www.aepd.es

2.    If you need to give power of attorney to a third party (acknowledgement of representation), you need to book an appointment by phone to 971763545 (Mallorca), 971398271 (Ibiza) or 971356554 (Menorca)

3.     On the day of the mediation:

a.  You must comply with the required protective measures. The use of mask and gloves is recommended.
b.  It is important that you arrive on time. In order to prevent overcrowding and possible delays in the mediation process, no grace time will be granted.
c.  In order to avoid overcrowding in the Tamib offices, only those that have been summoned will be allowed enter the premises, together with their legal representatives if so required
d.  You must produce your original ID card or passport when summoned (no photocopy or photo on your mobile phone will be accepted). You must also produce, if required, the company’s articles of incorporation, power of attorney for lawsuits, partnership deeds…
